Types of Clinical Reports We Write
Initial Assessment Reports
Documents assessment findings, functional strengths, support needs and recommendations.

Our reports build a functional understanding of your child’s nervous system and physical capabilities, and can be used as supporting evidence for NDIS planning, often aligning with or contributing to Functional Capacity Assessments (FCA).
Current Status / Progress Report
Reviews progress, current function, therapy goals and recommendations, including evidence that may support NDIS review planning.
Clinical Letters
- Medicare letters – documentation required for specific rebate pathways.
- Equipment letters – clinical justification for specialised tools or assistive technology.
- Mental Health Care Plan (MHCP) letters – acknowledgement of MHCP referrals.
- Chronic Disease Management (CDM) letters – first appointment confirmation letters.
- Letters to consutlants or other stakeholders
Educational Report
Documents observations and practical recommendations following a school, kindy or childcare visit.
Can an OT report support NDIS planning?
Yes. Our reports can provide clinical evidence about functional abilities, support needs and therapy recommendations for NDIS planning or review. The type of report required depends on the purpose and the evidence requested.
