Types of Clinical Reports We Write
Initial Assessment
This report is a comprehensive clinical assessment of your child’s strengths and support needs. It outlines our clinical reasoning and provides insight into the ‘why’ behind your child’s daily experiences.
Our reports build a functional understanding of your child’s nervous system and physical capabilities, and can be used as supporting evidence for NDIS planning, often aligning with or contributing to Functional Capacity Assessments (FCA).
Educational Report
Following a visit to your child’s school, kindy or daycare, we create a report designed to make their learning environment more accessible.
We provide educators and carers with practical strategies that match your child’s sensory and motor profile, and look at how the classroom setup and routines can be adjusted to support focus, participation and comfort.
Current Status Report
After a dedicated period of therapy, we reassess to see how your child has evolved. We compare their initial profile with current strengths and challenges to check that our approach is still effective.
The focus is to celebrate progress, update our clinical reasoning, and ensure the therapy plan still fits your child’s interests and needs. These reports are especially important for NDIS plan reviews to show the impact of therapy.
Clinical Letters
- Medicare letters – documentation required for specific rebate pathways.
- Equipment letters – clinical justification for specialised tools or assistive technology.
- Mental Health Care Plan (MHCP) letters – acknowledgement of MHCP referrals.
